7/10/2023 EnchantaVaughn Approved Behavior: Client displays age appropriate skills during one on one; avoiding conflict, staying on task and focusing on achieving her goals. The client was supported as she focused on the part of herself that she wants to get better. As the client was provided with a calm environment and reassurance that she would be cared for, her level of agitation has decreased. Client shows she is capable of being responsible and was supported for her contribution and cooperation in the home. Client reflected on her day, the good and the bad. Client communicated that she spends a lot of time helping her mom with different tasks at home, including watching her little sister, cooking, cleaning, and taking care of the dogs. Behavior: Client was excited to start the psycho educational activity, as evidenced by her smiling. Client sat quietly and waited for instruction. Client demonstrated good conduct, adhering to home rules and making a good first impression. Client displayed pleasant social behaviors during activity by keeping her hands to herself and staying focused. Client was polite and attentive at this time. Client displayed interest in session by debriefing and answering appropriate questions. As the client builds rapport with QMHP she was seen listening and giving positive feedback in her family discussion. Intervention: QMHP monitored client individualized behavior modification through collaboration with mother. QMHP talked to client about her confidence levels and how they can affect her day and self-esteem. QMHP praised mother for showing more consistency and efforts with assisting her daughter in understanding how to follow rules in the home and increase her social skills outside the home. QMHP praised client for her commitment in trying to meet her objectives. QMHP practiced more approaches that mother could use in order to make sure that client remained positive and happy in the home. Intervention: QMHP inquired about client’s afternoon activities and what she does when she gets home every day. QMHP offered active listening skills. QMHP educated and practiced with the client on using breathing techniques and other anger management skills to assist her with maintaining a calm spirit. QMHP assisted client in monitoring her own behaviors in efforts to promote exercising self-control in focused areas such as respecting boundaries with others. QMHP modeled proper positive behavioral expectations for the client. QMHP reiterated with mom the importance of maintaining a healthy relationship with her daughter by allowing client to make new friends and be her own person. Response: Client indicated that she was going to try her best to work with QMHP and be open to new things and promised to let mom or QMHP know when she felt overwhelmed in session. Client verbalized that she’s able to focus better when it is quieter and more peaceful. Client expressed excitement, demonstrating mindfulness and seemed to be motivated to observe closely in social environments. Client implemented good communication skills by listening attentively and closely. Client responded well to feedback, interacting well with everyone. Client agreed to behave at home more often. Response: Client displayed appropriate social skills during second part of the session. Client was sociable and friendly in regard to the QMHP support. Client was receptive to the education and training. Client gained knowledge about following directions the first time and being a good listener. Client demonstrated good social skills and ability to get along with others. Client expressed positive self-talk, modeled positive behavior, and good communication skills during role play activity. The interventions were noted successful and making a difference, as the client displayed new habits and routines.
